the dreams/nightmares are like pocket dimensions, your character is tied to the hunters dream during the events of the hunt so you just respawn instead of dying.
you can either be set free by gehrman, replace him as the new prisoner/caretaker of the hunters dream to guide future hunters during future hunts, or become a great one that replaces the moon presence

Pretty much. He's materialized back to the Waking World, but this time he's severed from the Hunter's Dream, i.e. your MC will never be able re spawn again. Read/listen to all the dialogue Eileen and Djura give you.

Sort of, there are no "good" ones in the traditional sense, but I like the slug ending. You save Gerhman, replace the Moon Presence, and become the first of your kind to truly ascend.
Its not true immortality so long you are chained to Yarnham. Which is why a lot of Hunters go bonkers and give up.
Which is why killing them stops them from respawning, since you are the aide of the MP just like Gehrman is.
No matter where you go in the game, you are still within a mere days travel of the Tombs.
Some of the weirder shit happens closer to the pocket dimension than the real world. I.e some of the fouler beasts migrating from the pocket dimension, triggering the lynch mob to take over the street.
I am not sure to the outside if you only exist as Nights of the Hunt, or if the Nights of the Hunt is unnaturally long to those inside of Yarnham.
all hunters make a packed with the moon presence to hunt beasts and other old gods, I don`t think the game ever explains what both the hunters and moon presence get out of this, technically hunters are immortal but that doesn`t seem to be a motivation for any of the other hunters you meet.
It is also implies that all old gods want to have children which is however almost impossible for them, so hunters could be a form of child subsidiary for the moon presence, however this theory falls flat once you realize that Gehrman mumbles in his sleep that being stuck in the hunters dream is incredible painful for him.
If the moon presence main motivation is having children it would interact more with the hunters and possible make their lives pleasant.
In the OP ending your character get`s released of the contract, meaning he now can die but at least he doesn`t have to go on fighting anymore either.
Bloodborne touches on some interesting concepts but they are not well fleshed out and just incredible poorly told to the player.
People will say that "this is the charm and you are just too stupid to get IT" but in reality it jsut comes down to From Software being to lacy to fully write down the story and implement proper storytelling devise.

The reason why the game ends is because you halted a ritual that attracted the MP, or as seen from the Waking World, the Red Moon (source of the unusually severe beast scourge on this particular Night of the Hunt), not because you fulfilled the MP's contract to kill gods.
