The real, unironic explanation is that the Iron Keep (and a lot of areas in Dark Souls 2 for that matter) was originally much bigger and you would have had to climb up the mountains in Earthen Peak and an enter through the front of the castle

If you think i'm bullshitting just re watch the original trailers for the game. Almost all the footage of the Iron Keep isn't in the game like this pic. Basically they made the area too big so they had to cut out all of the beginning of the Iron Keep and just send you inside with an elevator
